Identification of the intellectual property right you claim is infringed;
A specific description of the allegedly infringing product or content;
Your full name, contact email and mailing address;
A statement that you have a good-faith belief the use is not authorized by the rights owner;
A statement made under penalty of perjury that the information in your notice is accurate and you are authorized to act on behalf of the owner of the exclusive right that is allegedly infringed;
Your physical or electronic signature.
